ER Ordering Protocol

If you are an ER Provider and you need to order a brace at any time, then please call our emergency cell phone:  859-351-5500.  The on-call practitioner will ask several questions about your patient’s diagnosis, accident and anatomy including height and weight and deformity.  The on-call practitioner will need four documents faxed to both 859-237-7017 and 859-403-3249.  The FOUR required documents are:

1.  Demographic Sheet Including Updated Insurance Info and Patient’s Telephone Numbers

2.  Script Signed and Dated by the ER Provider

3.  Copy of the Medical Record of the Patient’s ER Visit Created by the ER Provider

4.  Copy of the CT or MRI Scan that Documents the Patient’s Diagnosis

The most common spinal braces ordered by ER Providers are Prefabricated Rigid LSOs and Custom Anterior-Opening TLSOs.  

To order an LSO, please put this on the script:  Prefabricated Rigid LSO (l0637).  In the medical record please state this:  I am ordering the patient a prefabricated rigid LSO for spinal stabilization, comfort and pain control.  The patient will wear the LSO when out of bed during activities for the next 10-12 weeks.  I will contact Bluegrass Bracing about fitting the patient at home.

To order a TLSO, please put this on the script:  Custom Anterior-Opening TLSO (L0482).  In the medical record please state this:  I am ordering the patient a custom anterior-opening TLSO.  The patient needs a custom brace because (please include unique details about the patient’s anatomy such as obesity, kyphosis, scoliosis, barrel chest, large bustline, etc.), which precludes them from wearing a prefabricated brace.    The brace is for spinal stabilization, comfort and pain control.  The patient will wear the TLSO when out of bed during activities for the next 10-12 weeks.  I will contact Bluegrass Bracing about fitting the patient at home.

Once Bluegrass Bracing receives the four documents, we will contact the patient and make arrangements for a home visit.  We cannot provide bracing services until we have started the insurance precertification process.  We are available 7 days a week and all hours of the day.  Please call us at any time if you have questions about which brace to order and whether or not a brace would be appropropriate for your patient.
